June 28th at 5:30am in the morning leaving from the East Hills Library parking lot - July 3 Late at night around Midnight.

The BEST week of your life of course!

- Chartered transportation to and from camp
- Six days at one of Young Life's premier camping properties.
- All meals while at camp are included. There will only be 3 meals to and from camp that are not included..
- An extravagant six days with your friends
- All rides and activities at camp are included in the price.

Health and Consent Form
1. Follow the "Cognito Health/Consent Form" Button
2. Complete the Cognito Form using the information below.
3. Follow the" Doctors Signature Form" button. Print this off and take to a doctor for them to complete.
4. Bring the completed "Doctors Signature Form" to the bus or send it to via email to [email protected]
You will need the following information to complete the form:
(Your sports physical for school will work if it is current within the past 12 months).

- Area Number - MO22
- Trip Leader email address - [email protected]
- Camp - Castaway Club
- Camp dates June 28 - July 3 – the dates of the camp week that you or your child is attending.
- Health insurance information – Insurance company name, policy number, mailing address and phone.
- Health history including immunization record.
(Your sports physical for school will work if it is current within the past 12 months).
